help with an entry form for cross country riding?

i am new to cross country riding, what does this form mean?

tell me what the costs are, i dont own any thing, not even a horse.

the top is self explanatory. Next, what is your riding level? green means you have never been trained, Novice is beginner, and so on. The next part wants to know what you want and how much it cost. Are you doing the schooling day? if so do you need a vest? You can ask for help from the officials to fill out the form. Equestrian sport is very expensive. Good luck.
